Treatment Options

Treatment Begins With the Diagnosis

There is no single treatment that is appropriate for every patient with piles, fissure or fistula.

At Ramesh Proctology, treatment planning is based on the diagnosis, severity of symptoms, anatomy of the condition, previous treatment history and the patient's overall health.

Some conditions may improve with lifestyle or medical treatment, while others may require a procedure or surgery.

Conservative and Medical Treatment

For selected patients, treatment may begin without surgery.

Changes in bowel habits, fibre intake, hydration and avoidance of excessive straining can play an important role in the management of haemorrhoids and anal fissures.

Depending on the diagnosis, prescription medication may also be used.

The aim is to treat the underlying problem while reducing symptoms and preventing repeated trauma to the anal canal.

Treatment for Piles / Haemorrhoids

Treatment depends on whether the haemorrhoids are internal, external or combined and whether symptoms involve bleeding, prolapse, swelling or pain.

Management may include bowel-habit modification, office-based procedures or surgery.

Not every patient with piles requires surgery.

Office-Based Procedures

Certain internal haemorrhoids can be treated using procedures such as rubber-band ligation, injection therapy or other techniques.

These procedures may be suitable for selected patients depending on the haemorrhoid grade and symptoms.

Surgical Treatment

Surgery may be considered for patients with larger prolapsing haemorrhoids, significant external disease, recurrent symptoms or conditions that have not responded adequately to other treatments.

Laser Treatment for Piles

Laser procedures are available for selected haemorrhoidal conditions.

They may provide advantages in short-term recovery for some patients, but laser treatment is not automatically the best option for every type of haemorrhoid.

The choice of procedure should depend on the anatomy and severity of the disease rather than the technology alone.

Treatment for Anal Fissure

Acute fissures may improve with measures designed to keep stool soft and reduce trauma during bowel movements.

For chronic fissures, treatment may include prescription medication, botulinum toxin treatment or surgery depending on the patient's condition.

Surgery is generally considered when symptoms persist despite appropriate non-surgical treatment or when clinical findings indicate that it may be appropriate.

Treatment for Anal Fistula

Anal fistulas often require a surgical or procedural approach.

The treatment selected depends on how the fistula passes in relation to the anal sphincter muscles.

Options may include fistulotomy, seton placement, sphincter-preserving procedures or other approaches selected according to fistula anatomy.

In recurrent or complex fistulas, imaging such as MRI may sometimes be required before treatment planning.

Proctology Surgery

Surgery may be recommended when a condition is unlikely to resolve with medical treatment alone or when symptoms significantly affect the patient.

The type of surgery, anaesthesia, recovery and follow-up vary according to the procedure.

Before surgery, patients should understand:

The diagnosis, why the procedure is being recommended, available alternatives, expected recovery and potential benefits and risks.

Individualised Treatment Planning

Two patients with the same general diagnosis may not need the same procedure.

Treatment selection can depend on:

Severity of symptoms, disease anatomy, previous procedures, general health, continence, medications and the patient's priorities regarding recovery.

The goal is to choose an appropriate treatment rather than simply the newest or most heavily advertised procedure.
